「お客さま情報等確認書」ご提出のお願い（Request for Submission of “Customer Information Confirmation Form”）
クレジットカードを引き続きご利用いただくために「お客さま情報等確認書」のご提出をお願いする書面を登録のご住所宛に6月中旬以降にお送りしています。ご返送いただけない場合、カードがご利用いただけなくなる場合がございますので、「お客さま情報等確認書」へのご回答、ならびにご返送をいただきたく、ご理解・ご協力の程お願いいたします。
（To continue using your credit card, we will send a letter to your registered address in mid-June or later, requesting the submission of a “Customer Information Submission Form”. Please note that if we do not receive your response, your credit card may become unavailable. We kindly ask for your understanding and cooperation in completing and returning the “Customer Information Confirmation Form”.）
お客さま情報等確認書とは（What is the “Customer Information Confirmation Form”？）
「お客さま情報等確認書」とは、マネー・ローンダリングおよびテロ資金供与対策の一環として、本人確認書類の最新化を依頼する書面であり、ご入会時に「在留カード」の提出があったお客さまに対して在留資格・在留期間（満了日）等を継続的に確認させていただきたくお送りしています。
（The “Customer Information Confirmation Form” is a document used to request updated identification documents as part of our anti-money laundering and terrorist financing measures. This document is sent to customers who submitted their Residence Card when applying for their credit card, to periodically confirm their residency status, period of stay (expiration date), and other relevant information.）
ご提出方法（How to Submit the Form）
登録のご住所宛に届いた、「お客さま情報等確認書」のご提出をお願いする書面をお手元に準備お願いいたします。
Please prepare the letter you received at your registered address, requesting the submission of the “Customer Information Confirmation Form” .
